Membership Terms and Conditions

    Chapter 1. General provisions
    Article 1. (Purpose)

    The purpose of this terms and conditions(“Terms”) is to stipulate any conditions and procedures required for using services provided by Phil Hospital of Korean Medicine(“The Hospital”), rights for the hospital and members, meaning or responsibilities.

    Article 2. Effect and change of terms and conditions
    1. 1.These terms are effective on disclosure to the members.
    2. 2.The hospital may revise or update these terms at any necessary times, and the revised terms will be effective in the same way.
    Article 3. Other rules

    If any unspecified subject is stipulated by related law, the hospital will follow as it specifies.

    Chapter 2. Signing up and using the service
    Article 1. Definition of “Member”

    By members, it means any individual the hospital recognizes as suitable, who agrees to these terms to complete membership sign-up forms to be issued with a website ID and a password.

    Article 2. Establishment of service membership
    1. 1.The service membership is established upon the hospital’s acceptance on the member’s sign-up form, and the member’s agreement on these terms.
    2. 2.Any individual who wish to sign up as a member to use the provided service should provide personal information requested by the hospital.
    3. 3.If a member's sign-up form is accepted by the hospital, the hospital will inform the member of the member ID, or any information that the hospital considers necessary.
    4. 4.The ID entered upon the sign-up process cannot be changed, and only one ID will be designated for one individual.
    5. 5.The hospital will not accept any sign-up form conforming to the cases below.

      1. 1. Forms requested with names other than one’s own
      2. 2. Forms requested with false names
      3. 3. Forms requested with any false information
      4. 4. Forms disrupting the social stability or order, customs
    Article 3. Service use and restrictions
    1. 1.Service use is available throughout the year, 24 hours a day, unless with business or technical difficulties.
    2. 2.Service use may be restricted after notice when necessary, such as system surveillance tests.
    3. 3.On-line counseling may not be available 24 days a day, under personal circumstances of the replying medical staff.
    Article 4. Payment for the service
    1. 1.Service use is free of charge for anyone who signed up as a member.
    2. 2.If the hospital is to charge for the service, the time, policies, and prices will be announced before the payment.
    Chapter 3. Withdrawal, re-enrollment and restrictions
    Article 1. Withdrawal from service
    1. 1.Members can always request for withdrawal from the service, and the hospital will execute the request immediately.
    2. 2.However, in case of withdrawal, the reasons for request should be filled out.
    3. 3.If the designated ID and password do not function, the withdrawal process if successfully finished.
    4. 4.After withdrawal, the ID and e-mail address cannot be used for re-enrollment, and any ID or data cannot be restored.
    Article 2. Re-enrollment for service
    1. 1.If any member withdrawn from the service under article 1 wants to re-enroll for the service, the member shall agree with these terms, fill out sign-up form, and be issued with a new ID and password.
    2. 2.The ID and e-mail address once used for sign-up cannot be used again.
    3. 3.If the new ID and password is available for logging into the service, the re-enrollment process is successfully finished.
    Article 3. Restrictions for service use

    The hospital may terminate the membership or suspend a member’s service use without prior notice, if the member commits any of the following acts.

    1. 1.When social order or customs are critically disrupted
    2. 2.When related to criminal acts
    3. 3.When service use is utilized for disrupting national and/or social interest
    4. 4.When using other person’s ID or password
    5. 5.When damaging other person’s reputation or interest
    6. 6.When one individual is signed up using two or more IDs
    7. 7.When disrupting others’ use, such as vandalizing the service
    8. 8.When any related law or the hospital's policies are violated
    Chapter 4. Restrictions for service responsibilities
    Article 1. On-line counseling
    1. 1.The hospital will prevent the service members or users’ counseling records from releasing onto any 3rd person other than the attending medical staff or service administrator. However, the hospital will be free from responsibilities regarding record leakage in cases following below.

      1. 1.When the record is leaked by the user's negligence on password protection
      2. 2.When the user has deleted records using the ‘delete’ function
      3. 3.When records are leaked due to natural disaster, or any other situation that the hospital cannot control
    2. 2.For comprehensive, appropriate answers for the member’s counseling, the hospital's attending doctor or medical staff can refer to the counseling record and answers.
    3. 3.Counseling records, after eliminating any personal information, may be used for the following purposes.

      1. 1.Academic activities
      2. 2.Publication through prints or CD-ROMs
      3. 3.Service samples including FAQs or recommended counseling
    4. 4.All counseling answers are subjective views based on each doctor's medical knowledge, and do not represent the hospital’s overall service views.
    5. 5.In cases below, on-line counseling services or part of the answers may not be provided.

      1. 1.When the same counseling is requested repetitively
      2. 2.When counseling requests contain any irrational expressions
      3. 3.When a member requests certain diagnosis through counseling
      4. 4.When counseling request is about medical expenses or medicine prices
    Article 2. Information service
    1. 1.The information provided through the service is broad and general, and is only intended for basic information. Any information or consultation provided through the service cannot be substituted for professional medical diagnosis. Any information or consultation is not meant for substituting professional medical diagnosis, treatment, or care. Any questions or concerns about a member's health condition should be visited and diagnosed by an actual doctor. Do not, at any case, ignore a doctor’s diagnosis, or postpone being diagnosed, treated, or cared by professional medical staff because of the information provided here.
    2. 2.The hospital does not recommend any specific tests, products, or treatments mentioned in the service. Any idea expressed in the service is a subjective idea of an individual doctor. The hospital is not responsible for any documents or counseling records provided by the service.
    3. 3.Decision to follow or accept any information provided by the service, or any idea provided by the doctor, other members or visitors, is completely up to the member’s choice. Therefore, the hospital is not responsible for any damages, injuries, or other disadvantages resulting from the utilization of products, data, ideas or descriptions presented to the member.
    Article 3. Third party links

    The hospital is not responsible for any transactions on goods or services provided independently by any third-party websites.

    Chapter 5. Obligations
    Article 1. Company's obligations
    1. 1.The hospital, unless there is a special occasion, allows members to use the service.
    2. 2.The hospital is obliged to provide continuous, stable service under these terms.
    3. 3.The hospital shall provide technical safety measures on protecting and maintaining the members’ personal information.
    4. 4.The hospital shall not send e-mails for profit purposes, when the members do not want them.
    Article 2. Obligations for information security
    1. 1.Members are responsible for the management of their IDs or passwords.
    2. 2.Members shall agree on receiving service-related e-mails.
    3. 3.When one’s ID is stolen, members should inform the hospital.
    4. 4.The hospital shall not sell or provide personally identifiable information to any unrelated third party without the member or user’s consent. However, the hospital may use the information voluntarily enrolled by the members on below cases.

      1. 1.Providing information to developers, that are necessary in developing new features, information, or services useful to the members
      2. 2.Providing advertisers with statistics on service members and groups (with all personal information eliminated)
      3. 3.When the company needs to execute user-preference advertisements or services
    5. 5.The hospital may use cookies in order to maximize the member's convenience on service use. Cookies are small text files used to identify the re-visiting users by the website, in order to help continuous, personalized service use. Cookies typically work by issuing each user with certain numbers that does not mean anything outside of the concerned website. Cookies do not intrude into the users’ computer system, and any files in the users’ personal computer remains safe. The hospital cannot prevent the service's advertisers or related third-persons from using cookies. If members do not accept the use of cookies, they can adjust the settings on the web browser on whether or not to accept the use of cookies. However, some services, especially personalized information, may need cookies to function properly.
    6. 6.The members’ information may be mutually provided or used between the hospital's service and its business partners.
    Article 3. Member's obligations
    1. 1.Members shall not allow any third parties to use their IDs or passwords.
    2. 2.Members are responsible for any consequences caused by poor ID or password management.

    Members should not do any of the below.

    1. 1.Using false information on membership sign-up or changes
    2. 2.Changing information posted by the hospital
    3. 3.Sending or posting information other than the hospital chooses, such as computer programs
    4. 4.Infringing intellectual rights, including the hospital and third parties’ copyrights
    5. 5.Any activities interfering with the hospital and third parties’ business, or damaging their reputations
    6. 6.Posting or disclosing any obscene, or violent messages, images, or media of any kind that disregards social order on the hospital
    7. 7.Trying to provide or recommend any medical diagnosis or counseling to other members
    8. 8.Trying to advertise a third party's medical treatment
    9. 9.Trying to sell, or recommend sales, any products
    10. 10.Any other activity that the hospital considers inappropriate

    If any of the above, or any other information that the hospital considers inappropriate is posted on the website or on linked third-party websites, the hospital may delete the information or stop the link from functioning without the member’s prior notice. However, the hospital is not obliged to delete or stop such information or links.

    Chapter 6. Noticing members

    The hospital may use e-mail address or phone numbers provided by the members when making notices to members. When notice has to be made to unspecified number of members, the hospital may substitute individual notices with posting on-line.

    Chapter 7. Medical reservations

    Members may apply for a medical reservation with the below information.

    1. 1.Name, date of birth, gender, contact information, address, e-mail address Enter
    2. 2.Choosing treatment clinics and dates
    3. 3.Agreements on the terms and conditions

    The hospital will accept reservation applications, unless it falls under any of the below.

    1. 1.When there is false, omitted, or misplaced information in the application
    2. 2.When acceptance is determined to be critically impaired in a technical way
    3. 3.The medical reservation is established when the member is informed with the hospital's acceptance through mobile text messages.
    Chapter 8. Copyright use

    Members may apply for a medical reservation with the below information.

    1. 1.Copyright and other intellectual property rights written by the hospital all belong to the hospital.
    2. 2.Members shall not use any information obtained by using the website for profit, including copying, sending, publishing, distributing, or broadcasting, or let any third party use it without the hospital’s prior consent.
    Chapter 9. Mediating disputes
    1. 1.Except as otherwise provided for in these terms, any disputes regarding the use of services shall be resolved by bilateral agreements.
    2. 2.In case of lawsuits regarding any disputes on service use, the competent court will be the court in charge of the hospital's location.
